Notice of Privacy Practices
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
This Notice is effective on April 14, 2003
Jersey Health insurance is committed to maintaining and protecting the confidentiality of the personal information of our clients. We are required by law to protect the privacy of your individual identifiable health information and other personal information and to send you this Notice about our policies and practices in this regard. We are bound by the terms of this notice when we use or disclose your confidential information. We may revise this Notice at any time.
This Notice of our privacy practices and duties associated therewith is mandated by law. We are covered by the law as Business Associates to entities who are considered covered under the law such as health insurance providers. Information that is protected under the law includes all information that we have received or created about past, present or future health or medical conditions and which otherwise could be used to identify you. The information also includes all medical treatments you have received, claims you have made and health care payments made on your behalf or on behalf of your employees. We are required to notify you as to how, when and why your personal information is being used. Our Privacy Notice is set forth in detail below. We would ask that you review it and that you share it with those individuals receiving coverage under your plan or policy. The term "customer" as it is used in this notice refers to individual claimants, insureds, beneficiaries or applicants. This notice details the types of information we collect, who we might share that information with, and the security measures we have in place to respect the privacy and confidentiality of the information we collect.
How We Protect Your Privacy - Protecting your privacy is important to National Association Services, Inc. and our employees. We want you to understand what information we have and how we might use it. In order to provide our customers with service as effectively as possible, we use technology to manage and maintain customer information. We will not disclose confidential information about our customers without authorization unless it is necessary to obtain health coverage on your behalf, provide your health coverage, provide your health benefits, administer your health plan, assist in reviewing coverage issues or making claims, or as otherwise required or permitted by law. When we need to disclose confidential information, we will follow our policies and procedures as set forth in this notice. This will be done to protect the confidentiality of our customers in the best manner possible. Our Policy at Preferred Benefits Group, Inc. is to keep your health records and information as safe and as confidential as we can.
I. Personal information we collect as Business Associates of Insurers:
There is a variety of information that we require and must collect in order to perform services for our customers in obtaining health coverage and benefits. This includes:
- Any information that is provided to us through the completion of various forms in the course of business including:
- Enrollment Forms
- Claim Forms
- Beneficiary designation/Assignment forms
- Any other form necessary to effectuate coverage, administer coverage, or administer and pay a claim.
- Any information from others that is necessary for us to properly process a claim, obtain insurance coverage, or to otherwise complete a transaction requested by our customer.
The information collected can include name, Social Security number, address, date of birth, phone number, marital status, gender, dependent information, bank account information, payment information, claims history, treatment information and employment information. While this list is not exhaustive it should give you an idea of the types of information we are referring to in this notice and that must be kept confidential.
II. How we use and disclose Personal information:
- We will not use your confidential information or disclose it to others without your authorization.
- We do not disclose any of the above-described personal information about our current and former customers to any third parties except as permitted by law, such as for completing a requested transaction, and/or if authorized by our customer.
- For various business reasons, we may need to provide our customer's personal information to various insurers or others, including a broker, third-party administrator, re-insurer, employer or plan sponsor. These disclosures may be made to others for the purpose of performing administrative services on our behalf, assisting us in obtaining coverage or benefits, helping us administer or review a claim, where we feel it necessary to protect our interests, to comply with workers compensation laws, as required by law or as requested by a governmental agency.
- We may disclose personal information to service providers and co-brokers in order for them to help us provide superior products and services. Should we determine that these disclosures are necessary, we will seek assurances that these third parties will not further share the information beyond its stated purpose.
- You may revoke an authorization that you have previously given by sending a written request to our Privacy Contact identified above. Any such revocation cannot apply to actions we have already taken.
III. Health information.
- We will not share any of our customer's health information unless allowed by applicable law and/or the customer has provided us the appropriate authorization.
IV. Confidentiality and Integrity.
- We restrict access to your personal information to those employees who need to know that information in order for us to provide our services to you.
- We use physical, electronic, and procedural controls, including physically secured areas and computer access controls.
- We assess the integrity of our systems through monitoring and data management processes.
- We have policies to direct and procedures to limit access of a customer's information.
- Our employees are trained on how to keep our customers' information safe.
